计划异常(inkos plan chapter xxx)

What happened? The web dialog uses natural language to list the plan for a chapter for me, and it always reports the following issues: "The model has output chat text that appears to be the body of the chapter, but no landing tool was invoked. InkOS will not treat the chat text as a saved chapter: to continue writing a new chapter, send 'Continue writing the next chapter'; to revise an old chapter, send 'Re-write/revise Chapter N + specific requirements', and the system will go through the reviser/writer pipeline for landing."
