Pull request template?

I just realized we don't currently have one. Could be nice to just help guide our contributors to write a well-structured PR, but also to assert they've read the contributing guidelines. A lot of PR templates I've seen have a checklist like this:

- [ ] I've read the contributing document
- [ ] I've added tests
